Suffer a lot and don't forget to work on your descending skills. More seriously, you need to do a lot of long, hard climbing. Depending on where you are you can mimic this with long tempo intervals.

I trained 30 people for last years Etape, the main areas that I got them to work on were long efforts and back to back rides. The Marmotte is very hard so people need to be prepared for a tough day and for the long time they will spend climbing. It isn't just a matter of sticking a bigger rear sprocket on and hoping for the best!
